Priority Patient Transport is a Virginia-based emergency and non-emergency medical transportation provider. We run ambulance and wheelchair transport out of six locations across the Shenandoah Valley and Central Virginia, serving hospitals, skilled nursing and rehabilitation facilities, hospice agencies, and non-emergency medical transportation brokers.

We are hiring a Billing Manager to take over our billing department. This is a remote, full-time role with real ownership: you will run the function, not just work inside it.

Why this role is open Our transport volume has grown and our billing operation needs a dedicated leader. Today a small team processes roughly 1,700 to 2,000 claims a month across more than 100 active payers. The pieces are in place — TraumaSoft, an established clearinghouse connection, clean payer setup — but the department needs someone to own the numbers, tighten the workflows, and drive cash.

If you have been the strongest biller on a team and have been waiting for the chance to run one, this is that job.

What you will own

The full revenue cycle.

Charge capture through final payment or write-off, across all payer types.

Accounts receivable.

You will own days in AR, the aging buckets, and the plan to work down older balances. Reducing aged AR is the single biggest priority in your first year.

Denial management.

Build the workflow that works denials by CARC / RARC reason code instead of one claim at a time, and feed the root causes back upstream to crews and dispatch.

A broad payer mix.

NEMT brokerages (Modivcare, MTM, SafeRide, Access2Care, MediDrive), Medicare fee-for-service, Medicare Advantage, commercial carriers (Anthem / BCBS, UnitedHealthcare, Humana, Aetna, Cigna, Sentara), Medicaid MCOs, VA Community Care, direct facility contracts, and patient self-pay.

Broker portal reconciliation.

Trip authorizations, submitted runs, and payment files tied back to what we actually transported.

Facility and contract billing , including monthly invoicing and rate reconciliation for hospital and skilled nursing accounts.

Documentation compliance.

Physician Certification Statements, medical necessity, signatures, and the chase process that keeps claims clean on first submission.

Reporting to ownership.

You will build and own the dashboard: clean claim rate, first-pass resolution, denial rate, days in AR, net collection rate, and cash per transport.

The team.

You will supervise our billers, set standards and productivity targets, train, and hire as we grow.

Payer enrollment , revalidations, and provider credentialing support.

Month-end close

in coordination with accounting.

What we require

Five or more years in medical billing, with at least two years in ambulance, EMS, or NEMT billing specifically. This is a hard requirement — ground transport billing does not work like physician or hospital billing.

Demonstrated supervisory or lead experience: you have managed billers, set work queues, or run a department.

Working command of ambulance coding: HCPCS A0425–A0434, origin and destination modifiers, ALS / BLS / SCT levels of service, loaded mileage, and Medicare rules including sequestration.

Hands‑on experience with 837 claim submission and 835 remittance posting, and comfort reading an EOB or ERA line by line.

Proven AR reduction. Be ready to tell us a specific aging you inherited, what you did, and what it looked like when you handed it over.

Strong spreadsheet skills. You should be able to pull a report and tell us what it means without waiting on someone else.

Reliable high-speed internet, a private home work space, and availability during Eastern business hours.

Nice to have

TraumaSoft experience.

If you have it, say so in your first line — it moves you to the top of the pile.

Virginia Medicaid and Virginia MCO experience.

NEMT broker portal experience (Modivcare, MTM, SafeRide, Access2Care).

CAC, CACO, CPC, or CPC-A certification, or an ambulance-specific credential.

Experience standing up reporting or KPIs where none existed.

Experience through a billing system conversion or a clearinghouse migration.
