Uplift Coach (QBHS/QMHS) provides community-based mental and behavioral health support, helping Clients to build skills, follow treatment plans, and access needed resources to stabilize, recover, and thrive.
Reports To: Clinical Manager (clinical) / COO (administrative)
Employment Type: 1099 Contractor
- Deliver CPST and related behavioral health services via telehealth, reinforcing treatment goals developed with licensed clinicians.
- Support Clients in developing coping skills, daily living skills, and problem-solving strategies using a strength-based, recovery-oriented approach.
- Coordinate care with therapists and other team members, assist with treatment planning, and link clients and families to community resources.
- Maintain timely, accurate documentation of contacts, progress (SIRP) notes, and service plans in accordance with Ohio Department of Behavioral Health and payer requirements.
- Participate in required training and supervision to maintain QBHS/QMHS competencies in mental health, crisis response, de-escalation techniques, and all other required training per OAC.
Final Accountability: The Uplift Coach is accountable for delivering compassionate, effective, recovery-oriented CPST services and maintaining timely, accurate documentation.
Qualifications:
- Required: Minimum of a high school diploma or GED, plus required behavioral health competency training/ State certification within 90 days if a behavioral health field degree has not been obtained.
- Demonstrated understanding of mental illness or substance use disorder treatment and recovery, community systems, and crisis response obtained through required training or certification training.
- Preferred: Associate or bachelor's degree in a related field (social work, psychology, human services).
- Lived experience or prior experience in behavioral health, peer support, or case management.
